Incident reporting allows for a log recording of event occurrences; including the date, location, what the incident was, and action items required to resolve the issue. the initial records can be logged by team members through the Employee Portal, or through the following process.
This feature can be accessed from the HR > Incident Reports menu item.

Recording New Incident Reports
The process for recording new incident items is similar to the employee lodgement process as outlined in the Recording Workplace Incidents article, with the exception that the employee records are pre-set to the logged in user, and the administrator record allows this to be selected.
To start the process use the Actions > Add Incident.
Here you can enter the details for the incident
NOTE: All fields on this screen are considered as mandatory.
Use the Save button to confirm the process.
Advanced Settings
With the incident item created the record can be opened and additional information can be added.
Incident Details
This tab shows all of the detail that was recorded in the initial step, as well as allowing the administrator to set whether the incident
- Is notifiable
- Requires monitoring
- Requires feedback to the people involved
- Its open status
- If there are any other incidents
Actions
The actions tab allows for a space to enter the steps that the company is taking to rectify the situation.
You are able to indicate the status, expected target date, the item closed date as well as the notes relating to the action
Monitoring
This tab allows for a space where you can input monitoring and review steps. It also allows for the upload of files which could be used to record medical\legal documents relating to the incident.
Multiple monitoring items can be added.
Notification
The notification tab allows for a space to input details for serious issue such as serious injuries, dangerous incidents, illness or death. You can enter the people or parties involved, further actions taken, and the preliminary cause of the incident.
